SmartClaw™ Thrombectomy Catheter

SmartClaw is an adjunctive thrombectomy device that is atraumatic to the vessel wall, but robust enough to clear organized thrombus.

In deep vein thrombosis (DVT) cases, the nitinol basket can be deployed distal to the thrombus, expanded circumferentially to fully conform to the vessel wall atraumatically, and pull the thrombus back to the catheter with its unique weave.

Case study

SmartClaw led to a successful outcome in a DVT mechanical thrombectomy case.

The patient had previously placed bilaterial iliac VENOVO™ stents with VENOUS WALLSTENT™s extending through the common femoral veins. ​

The patient was placed in the prone position and left popliteal vein access was obtained.  ​The venogram showed the right side was open, but the left side was occluded from the common femoral vein through the iliac vein.  ​

The physician pre-dilated the left common femoral and iliac veins with a Mustang™ balloon to open a small channel and achieved true lumen inside of the stent.  ​After IVUS, the physician used a 16FR Penumbra Lightning Flash™  and 32mm SmartClaw basket:​

  • SmartClaw was advanced coaxially through Lightning Flash and deployed above the stents​
  • The physician made two passes with the Lightning Flash catheter advancing from the common femoral vein to the IVC​
  • The Lightning Flash catheter was retracted down to the common femoral vein​

The SmartClaw basket was opened inside the stent and retracted for a total of 3 passes through the iliac and common femoral veins while aspiration with the Lightning Flash catheter was on​.

The thrombus was removed, and full flow was restored (shown in the Post-SmartClaw image).  An additional Wallstent was deployed to reline the stents and extend into the common femoral vein.

The physician leveraged mechanical thrombectomy with SmartClaw to successfully remove aged thrombus in a DVT intervention. After initial aspiration, the SmartClaw 20mm basket was traversed contralaterally.  The physician made 5 total passes in the femoral vein, which cleared residual, wall-adherent thrombus. Thrombus in the yellow highlighted box in image 4 came after the initial pass with Penumbra, showing the additional resolution with SmartClaw.
