Small Vessels 

Percutaneous coronary intervention of small-vessel coronary artery disease (SVD) is challenging because of difficulties with equipment delivery and i.e. restenosis rates, and drug-coated balloons are an attractive emerging PCI option for patients with SVD.

Drug-coated balloons are a novel concept for the treatment of coronary artery disease and while they are an established therapeutic option for in-stent restenosis (ISR) of BMS and DES, there is growing evidence, such as the randomized non-inferiority Basel Kosten Effektivitäts Trial–Drug-Coated Balloons versus Drug-Eluting Stents in Small Vessel Interventions 2 (BASKET-SMALL 2) trial, that the use of DCBs compare very favourably with second-generation DES.

DCB’s are based on the fast delivery of highly lipophilic drugs to the vessel wall after single balloon inflation and to overcome the limitations of elastic recoil and flow-limiting dissections after balloon angioplasty, optimal lesion preparation is also essential.
