Here are a couple of reminders for you to keep in mind as you return explanted and non-implanted product:

  • Be sure to follow the instructions contained in the kit with regard to packaging and shipping any biohazard devices.
  • Complete the return device form with any performance observations.
  • More than one device may be sent in a return kit.
  • Kits contain all necessary documents required to assure devices are returned directly to Guidant /Boston Scientific CRM.
  • Other manufacturers' products should be returned to their return product departments.
