Be sure to follow the instructions contained in the kit with regard to packaging and shipping any biohazard devices.

Complete the return device form with any performance observations.

More than one device may be sent in a return kit.

Kits contain all necessary documents required to assure devices are returned directly to Guidant /Boston Scientific CRM.

Other manufacturers' products should be returned to their return product departments.
