Order a Returned Products Kit

Returning products for analysis and providing product performance observations helps drive reliability higher on an ongoing basis. Please contact Technical Services (1.800.CARDIAC or 1.651.582.2698) at any time to report device related performance observations. Your role is an important part of the continuous product improvement process.
To order a Returned Product Kit, please send an email requesting a model 6499 Returned Products Kit with a shipping address and contact phone number to Boston Scientific CRM Customer Service, or call 1-800-CARDIAC.

Here are a couple of reminders for you to keep in mind as you return explanted and non-implanted product:

  • Be sure to follow the instructions contained in the kit with regard to packaging and shipping any biohazard devices.
  • Complete the return device form with any performance observations.
  • More than one device may be sent in a return kit.
  • Kits contain all necessary documents required to assure devices are returned directly to Boston Scientific CRM.
  • Other manufacturers' products should be returned to their return product departments.
