담췌관 내시경 기반 정밀 진단·치료 지원…임상 근거 바탕 건강보험 적용 확대

보스톤사이언티픽은 담췌관 내시경 카테터 ‘스파이글래스 DS 카테터(SpyGlass™ DS II Direct Visualization System)’가 보건복지부 고시에 따라 8월 1일부터 건강보험 급여가 확대^[1] 적용된다고 밝혔다. 이에 따라 환자 본인부담률이 기존 80%에서 50%로 낮아져 고난도 담관 및 췌관 질환 환자의 경제적 부담이 완화되고 필요한 진단 및 치료에 대한 접근성이 개선될 것으로 기대된다.   

급여 적용 대상은 동일하다. ▲1차 내시경적 역행성 담췌관 조영술(ERCP)만으로 확진이 어려운 담도 및 췌장 병변의 진단 검사 ▲1차 시술로 제거되지 않는 담석 및 췌석 제거 ▲담관 및 췌관 협착으로 가이드와이어가 통과되지 않아 추가 진단 또는 치료가 어려운 경우에 적용된다.[2]

스파이글래스 DS 카테터는 간관을 포함한 담췌관계의 내시경 시술을 진행하는 동안 담관 및 췌관 내부를 시각화해 의료진이 병변을 직접 관찰해 진단과 치료에 적용할 수 있도록 고안된 일회용 담췌관 내시경 카테터다. 연구에 따르면 ERCP 후 시행한 폐쇄 담관조영술에서는 확인되지 않았던 잔석이 담관 내시경을 통해 전체 96건의 시술 중 34%에서 추가로 발견됐다.[3] 또한, 담관 내시경 유도 조직검사에서 첫 번째 채취 검체의 진단 민감도는 68.2%로 ERCP 유도 브러시 채취의 21.4%보다 유의하게 높았다.[4]

이 밖에도 연구에 따르면 담관 및 췌관 내시경 검사 결과를 바탕으로 전체 환자 118명 중 34%에서 수술 계획이 변경됐다. 담관 환자의 25%는 수술을 피했으며, 췌관 환자에서는 31%에서 수술 범위가 확대되고 31%에서 수술 범위가 축소되는 등 치료 의사결정에 영향을 미친 것으로 나타났다.[5]

문종호 대한췌장담도학회 이사장(순천향대학교 부천병원 병원장 및 소화기내과 교수)는 “스파이글래스 담관내시경 검사는 고난도 담관 질환의 진단과 치료를 위해 다수의 국제 진료지침[6]^,[7]에서 권고되고 있는 검사인 만큼 이번 건강보험 급여 확대는 환자의 경제적 부담을 줄이고, 의료 접근성을 높인 매우 의미 있는 진전”이라며, “고난도 담관, 췌장 질환으로 시술이 필요한 환자들은 여전히 상당한 비용 부담을 안고 있는 만큼 향후 임상적 유용성과 실제 의료현장의 요구를 충분히 반영해 급여 범위와 적응증이 더욱 확대되기를 기대한다”고 말했다.
