간암 치료 위한 동맥경유 방사선색전술 의료기기 ‘테라스피어’, 아시아 국가 중 처음으로 건강보험 급여 적용

보스톤사이언티픽은 자사의 동맥경유 방사선색전술 (TARE, Transarterial radioembolization) 에 사용되는 간 종양 치료 의료기기 ‘테라스피어 (TheraSphere™ Y-90 Glass Microspheres) ’의 국내 건강보험 급여 적용 5주년을 기념해 기자간담회를 개최했다.

테라스피어는 방사선 동위원소인 이트륨 (Yttrium-90) 을 담은 미립구 (microsphere) 로 이뤄져 있어 간동맥을 통해 주입되어 종양을 직접 괴사해 암을 치료한다. 국소적으로 작용해 종양 주변의 정상 세포와 조직에 대한 방사선 영향을 줄여준다. [1] 2015년 6월 식품의약품안전처로부터 최초 허가를 받았으며, 테라스피어를 이용한 동맥경유 방사선색전술은 아시아에서 처음으로 건강보험 급여가 적용돼 2020년 12월부터 환자 본인 부담률이 절반으로 줄었다.

이 날 간담회는 ‘동맥경유 방사선색전술을 중심으로 보는 간암 치료의 현재와 미래’를 주제로 진행됐으며, 김윤준 서울대학교병원 소화기내과 교수가 좌장으로 참여하고 김효철 서울대학교병원 영상의학과 교수가 발표를 맡아 급여 등재로 변화한 치료 환경과 테라스피어의 국내 연구 최신 결과 및 임상적 가치에 대해 소개했다.

김윤준 교수는 테라스피어의 급여 5주년의 의의를 설명하면서 “간암은 재발이 흔하고 국내 암 사망 원인 2위로 사망률이 높아 세밀한 치료 전략이 필요한 질환”이라며, “방사선색전술은 급여 등재 이후 기존 치료를 보완하고 약물 치료 및 수술과 연계해 활용되며 치료 결과와 삶의 질 향상 등으로 환자와 가족의 전반적인 만족도를 개선해왔다”고 설명했다.

연자로 나선 김효철 교수는 “동맥경유 방사선색전술은 기존 화학색전술에 비해 색전후증후군이 적고 입원기간이 짧으며, 종양의 크기가 큰 환자에서의 치료 효과가 확인되면서 급여 등재 이후 환자 사례가 늘고 있다”며, “방사선색전술이 중기 혹은 말기 환자뿐 아니라 초기 간암 환자에게도 유용하다는 연구가 진행되면서 테라스피어가 다양한 병기와 종양 크기의 간암 환자에게 치료 옵션이 될 수 있다는 가능성도 확대되고 있다”고 말했다.

김효철 교수는 이어서 테라스피어를 통한 매우 초기 또는 초기의 한국인 간암 환자 154명을 대상으로 진행한 연구자 주도 임상 연구 (Korean multicenter prospective observational study on efficacy of Y90 for HCC, KURE-YTT-HCC) 의 업데이트 된 중간 결과를 소개했다.[2] 김 교수는 “최대 29개월까지 추적 관찰한 결과, 객관적 반응률 (ORR, Objective Response Rate) 이 97.9%였으며, 그 중 표적 종양이 모두 제거된 환자 비율 (CR, Complete Response) 은 87.6%, 표적 병변이 30% 이상 감소된 환자의 비율 (PR, Partial Response) 은 10.3%로 나타났다”고 설명했다. 또한, “기존 글로벌 연구와 달리 초기 환자를 포함하고 있다는 점과 국내 환자의 특성을 반영했다는 점에서 의미가 크다”고 덧붙였다.

정애리 보스톤사이언티픽 한국 총괄 대표는 “테라스피어가 2020년에 아시아-태평양 지역에서 처음으로 우리나라에서 급여를 받은 것은 의료진과 환자에게 더 나은 결과를 위한 새로운 선택지가 주어진 의미”라며, “앞으로도 보스톤사이언티픽은 국내 및 아시아 환자에게 필요한 치료 옵션을 제공할 수 있도록 최선의 노력을 다할 것”이라고 말했다.

테라스피어 방사선색전술의 매우 초기 또는 초기의 간암 환자 대상으로 진행한 연구자 주도 연구인 ‘KURE-YTT-HCC (책임 연구자: 김윤준) ’의 97명의 환자를 최대 19개월 추적 관찰한 중간 결과는 국제학술대회 ‘리버위크 2024 (The Liver week 2024) ’와 ‘제14회 아시아 태평양 간암 전문가 회의 (APPLE 2024) ’에서 발표됐다. 154명의 환자를 최대 29개월 추적 관찰한 추가 결과는 지난 4월 제19차 대한간암학회 정기학술대회에서 발표됐다.
