CMS Hospital Inpatient New Technology Add-on Payment (NTAP) 

Boston Scientific continues to lead the effort to secure appropriate coding and payment for EXALT Model D.    

  • Medicare Hospital Inpatient New Technology Add-on Payment (NTAP): On October 16, 2020, Boston Scientific submitted an application for NTAP under Medicare’s Hospital Inpatient Prospective Payment System. If approved, this device payment will apply to single-use duodenoscopes when treating Medicare patients in the hospital inpatient setting for eligible inpatient admissions. The Centers for Medicare & Medicaid Services (CMS) will make a final determination on the application in 2021. 
  • Inpatient ICD-10-PCS Code: CMS is proposing to approve Boston Scientific’s application for a unique ICD-10-PCS code to support hospitals in reporting the use of EXALT Model D, during ERCPs performed in the inpatient setting. 

Commercial payers are not required to follow CMS coverage; however, some may choose to do so. It is recommended to reach out to commercial payers to understand commercial payer reimbursement for EXALT Model D.

CMS Approves Transitional Pass—through Payment Category

 

 
The Centers for Medicare & Medicaid Services (CMS) has approved Boston Scientific’s application for a transitional pass-through payment category to describe single-use endoscopes, such as the EXALT™ Model D Single-Use Duodenoscope, with the intent to facilitate Medicare patient access to these new and innovative devices. Effective July 1, 2020, the new pass-through code, C1748,  may be used to bill for EXALT Model D when treating a Medicare patient in the hospital outpatient setting.

In a consecutive case series of the EXALT Model D Duodenoscope, 60 ERCPs, of all 4 ASGE complexity levels, were completed across six sites in the United States for indications including pancreaticobiliary stone clearance, stent placement, exchange and/or removal and balloon dilation.1

There have been more safety communications on duodenoscopes than any other medical device since 2013. The environment, equipment and guidelines that inform cleaning procedures are constantly changing putting duodenoscope reprocessing into a state of transition. Recently, the FDA recommended healthcare providers transition to duodenoscopes with disposable components or fully disposable devices.
