Providing Advanced Cardiovascular Care to All Americans

 

In June 2006, a team of physicians and health care professionals from across the country came together to look at ways to address disparities in cardiovascular care for the underserved patient populations of women, black Americans, and Latino Americans. From these discussions, Close the Gap - an educational initiative sponsored by Boston Scientific - was launched. 

In an effort to identify opportunities and define strategies to improve cardiovascular health outcomes for these underserved patient populations, the Close the Gap Steering Committee created a "proof of principle" pilot program. The pilot program was launched in 10 cities strategically selected based on demographics and other defining criteria.

 

Community Education

 

The goal of our community education focus is to increase awareness of cardiovascular risk factors through a variety of community programs. By increasing awareness of risk factors, we hope to identify - and help - more high-risk patients. Our efforts to achieve this goal include partnering with community institutions including faith-based organizations, collegiate and professional athletic organizations, and patient advocacy groups. 

An example is the strong and valued relationship Close the Gap has forged with the Black Coaches and Administrators (BCA). The BCA is a national nonprofit organization whose members are highly influential mentors and leaders in their local communities. Because of their visibility, many BCA members serve as role models for the ethnic groups they represent. Our partnership with the BCA has afforded us new and creative venues for educating key audiences about cardiovascular health. BCA has facilitated opportunities for Close the Gap Steering Committee physicians to address decision makers within the NCAA, NFL and other major sporting organizations. 

In addition to special awareness events, several unique tools and educational materials have been developed to support our community outreach efforts.
