Instructions

Pacemaker, defibrillator, and heart failure patients: Check the Medical Device Identification (ID) card that you were given when you received your implant. You need to know the model number of your pacemaker, defibrillator, or heart failure device (for example, 1180). If you do not have a Medical Device ID card, you can order an ID card on our website. If you have a device model manufactured by Guidant Intermedics, please contact us through the website or call Guidant at 1-866-GUIDANT (1-866-484-3268).
