Charitable Contributions

Boston Scientific provides financial support for charitable purposes through the Boston Scientific Foundation. Information about the Boston Scientific Foundation may be obtained at the company's website (www.bostonscientific.com).Boston Scientific divisions may also provide financial and product support to qualified non-profit 501(c)(3) designated charitable organizations to support the following types of charitable programs:

• Fund raising dinners
• Other charitable fundraising events
  
  Note:  Requests for charitable donations to organizations outside of the health care industry (e.g., scouting, youth programs, private schools, museums, fine arts programs) are outside the scope of giving by Boston Scientific divisions, but may be eligible for consideration by the Boston Scientific Foundation. 

Boston Scientific CRM provides charitable contributions for the purpose of furthering indigent care, patient education or public education of scientific knowledge and discourse in the fields of medicine in which Boston Scientific has a business interest.

Required Documentation

1. Completed Charitable Contribution Application form.
   
   
2. Letter of request on institution letterhead which includes program date and location; an explanation of how the funds will be used and/or who are the beneficiaries; type of support requested (i.e., money or free product).
   
   No-Charge Product Donations
   For requests for no-charge product donations, the request letter must state that the patient is indigent and neither the institution nor the physician will charge for the procedure.
   
   
3. Copy of the fundraiser brochure detailing the sponsorship levels and fundraising purpose.
   
   
4. Proof of 501(c)(3) designated status.
   
   
5. Completed W-9 form signed by the institution.

Please note: Our divisional grant committees cannot review incomplete submissions.

It is possible for any submission to be approved at an amount less than the amount requested or for a submission to be completely denied due to budgetary constraints. Past funding is not a guarantee for a future approval. It is important not to assume approval until a formal confirmation of approval has been provided. Submissions may be referred to another Boston Scientific division for possible consideration for funding.
