Guidant Medical Device ID Card. This card helps identify you as a patient with an implanted medical device (pacemaker, defibrillator, or lead wire). It includes your doctor's name and telephone number and other important information. You should carry your Guidant Medical Device ID Card with you at all times. Your doctor or nurse should have given you a temporary ID card when you had your implant. You should have received a permanent ID card from Guidant about 8 weeks later. If you did not receive a permanent ID card after 8 weeks, or you need a new one, call us at 1-866-484-3268 to order one.
Medical Device Patient Travel Card. If you plan to travel, you may also want to order a Medical Device Patient Travel Card. You can show the card to airport security staff. The card explains that your implanted device may trigger airport security alarms. We also include the information in French, German, Italian, and Spanish. If you need the information in a language other than those listed, let us know. Call 1-866-484-3268 to order a patient travel card.
