The National Health Disparities Initiative was launched in 2003, with three-year grants to support community health centers. By developing models that focus on patient self-management, we've helped improve the health outcomes for patients with diabetes, cardiovascular disease, hypertension, stroke and pediatric obesity.
Phase two of this initiative, launched in 2007, targeted the health of homeless patients and migrant farm workers. Through this ground-breaking program, clinicians across the country have been trained on how to realistically set self-management goals with homeless patients.
The Boston Scientific Foundation has contributed more than $2 million in grants to help address health disparities with a focus on non-profit organizations in Northern California, Minnesota and Eastern Massachusetts. This initiative does not accept unsolicited requests.
